FARE-Community Needs Assessment

We are part of the FARE Centers of Clinical Excellence network. FARE (Food Allergy Research and Education) has taken to a more local level and is interested in what are our community needs to take care of our food allergic population. To accomplish this, there is a survey that can help. The note that follows is a request from FARE and a link to participate in the survey. PLEASE take a moment and do this.
Note that this is for the greater Indianapolis area-

Hope you are doing well.  As of yesterday at 4:00pm. Indianapolis only had 15 surveys completed.  I was hoping you could share the link to the survey with your fellow allergists and patients in Indianapolis.  Let me know if you have any questions about distributing the surveys.

Here is a sample email you can use for general dissemination:

Do you want to help drive food allergy awareness and education in your region? As part of FARE’s new community engagement initiative, we are developing a multi-year plan for the greater Indianapolis area. The plan will directly address issues facing individuals with food allergies and their healthcare team. You can help identify the issues by taking a short survey. The survey is open to parents of children with food allergies, adults with food allergies and medical professionals.  The survey will take you less than 10 minutes and is open through Friday, December 23.

Thank you for sharing your insights.
